CHEST PAIN: When should You Worry? (2019 Update)


hey this is dr. Barry let’s talk for just a few minutes about which kinds of chest pain you should worry about in which kind you can get to when you get to it chest pain can be a very concerning symptom and you should be concerned about it but you don’t want to be running to the emergency department for every little tweak you feel in your chest but at the same time and much more importantly you don’t want to stay home because you think your chest pain is no big deal so I want to go through the different types of chest pain you might experience and when you should go to the emergency department or see your doctor now if you have any doubt whatsoever obviously called 9-1-1 and if you’re having chest pain right now why are you watching this video call 9-1-1 okay because you haven’t watched this video yet so you don’t know the difference so don’t take the time to watch this video initiate your emergency medical services now okay now if you like videos that help you with your health and your nutrition one of the very first to note now let’s talk about chest pain so you’re not wired the same in your chest that you’re wired out in your your fingers and toes if I cut you right here you’re gonna hurt right there it’s not like that in your chest you can be having a severe lung problem or heart problem and just kind of hurt all over I have a vague uneasiness or pressure you’re not wired the same and so that’s why it can become so confusing with chest pain and then also if you have other problems like diabetes or previous heart disease lung disease then that even Muddy’s the water even further as you’ve probably seen in the media women don’t always have the same classic signs that most men have not all men but most and so for women also women tend to ignore things because they’re trying to probably do something to take care of you know their man and so they ignore themselves and you should not do that if you’re having chest pain you need to listen to this video think about this and and get emergency services or go see your doctor if you’re concerned about your chest pain don’t put that off okay three types that you don’t have to worry about right off the bat first if an exercise gets I mean if if chest pain gets better when you exercise then it’s probably nothing to worry about almost no serious chest pain is going to get better when you exercise and so if you go for walking that makes it better or you just get on the rowing machine or whatever and it go it goes away that’s not anything to worry about the next is if you have a pinpoint pain in the chest almost never is this a problem if you say if you can say oh it hurts right here right here or right here that’s almost never a pain to be worried about now you may still need to see your doctor if it continues and really bothers you but it’s probably not your heart which is what we’re always worried about if you have a pain that is a shooting pain or like an electrical shock or it’s like oh I just had a it was just for a second and it went from here to here or here to here or here to here doesn’t matter from where to where but if it’s a shooting quick pain like that that’s virtually never a problem or anything to worry about okay and there so those are the ones if you had and we all had those from time to time I just had one just in it’s nothing to worry about don’t worry about that unless it continues and gets worse than of course see your doctor now let’s talk about the types of chest pain you should worry about these can these can be a big deal these can be life endures and so you have to take these seriously yes even you women who are busy trying to do chores for you or whatever you got to take these things seriously these are not something you can ignore safely so if you have chest pain with shortness of breath and so any chest pain you’re like yeah I’m also out of breath or you know usually I can take this flight of stairs and it’s no big deal but now I have this discomfort this pressure and I’m more short of breath now I mean immediate you got to go see the doctor that could be bad that could be your heart or some heart lung combination don’t wait so your doctor next is if you have chest pain that you also have swear in your ankles in your feet in your legs it’s like I yeah I don’t know my chest has been even vaguely uncomfortable and I have all this swelling in my feet now immediately go to the doctor right now that’s that’s hard until proven otherwise and so chest pain was shortness of breath or chest pain with new swelling and your feet and ankles and legs all bets are off go to the emergency department and get checked out okay now what if it’s just the chest pain typically that we worry about is if you have a a squeezing pain or pressure like an elephant sitting on my chest or it feels like somebody’s got me in a bear hug and I can’t get a deep breath and I really feel pressure and tightness in my chest that’s the classic engine chest pain that doctors worry about don’t hesitate go see the doctor right then if you have that tightness or that pressure and that’s kind of the universal sign of this like I don’t know it just like something to go to the emergency room you need to see a doctor for that and it may turn out not to be your heart but for that kind of pain you’re a dummy if you don’t go to the emergency department okay and now let me tell you this you may not know this about me but but for the first few years of my practice I was an emergency room physician I took care of all comers at the local emergency room and so I was the guy and so never feel like you’re going to inconvenience the emergency department staff the doctor there or you’re gonna embarrass yourself if you are of appropriate age and you haven’t been here 12 times and you’re you’re 12 years old we’re not gonna be upset with you if you come to the emergency department with chest pain no I don’t work in the emergency department anymore but I can tell you from my experience never did we get upset with somebody who came to the emergency department with chest pain of any kind because we know that’s very scary and we know that has to be ruled out that it’s not something serious so don’t worry about what the nurse is gonna say or the doctor is gonna think none of that crap matters if you’re having chest pain that worries you go to the emergency department okay now the next one is if the if the chest pain gets worse with exercising when I say exercise I don’t mean you know running on the treadmill at full speed I mean that if you’re having chest pain that usually you carry your little bag of groceries up one flight of steps and you don’t have any problem now that causes chest pain that’s a problem that’s a red flag you need to see your doctor ASAP if you if you’re like you know just walk across the street now and I have this pressure you need chest pain that gets worse with walking with taking a flight of steps with exercise that is angina or stable angina until proven otherwise and you know the way to prove otherwise is to go see the emergency physician or your regular doctor ASAP and get that checked out if you had and so we already talked about the aching and the crampy deep kind of pain chest pain that can kill you almost never is a sharp shooting pain or just a sharp short-term pain it’s a deep visceral ache and so that’s the kind of pain that you should really be worried about now with the two exceptions one is if you’re a diabetic all bets are off because one of the things that diabetes does is it causes neuropathy and now diabetics know what neuropathy and their feet feels like it’s that numb tingly ant crawling up your sock discomfort but you can also have neuropathy and these nerves as well and so you might very well not have any of the classic Fred Sanford Oh Elizabeth I’m coming to join you you might not have any of that at all and I’ll tell you one example years ago working in the emergency department I had a little lady come in who’s a severe and controlled diabetic and she came in because she couldn’t stop throwing up she had severe nausea and vomiting that was her only symptom and she wound up having a massive heart attack and we didn’t save her and so she had waited hours before she came in but if she had known that as a diabetic if you have any discomfort from your nose to your navel all the way out to your fingers anything in from the nose to the navel if you have discomfort you’re a diabetic especially an uncontrolled diabetic or a newly diagnosed diabetic a diabetic who’s not eating the ketogenic way of eating you got to go to the emergency department to make sure that’s okay and so just remember my little lady that came in with nausea and vomiting and now is no longer with us because of the severe heart attacks she was having the other group or class of people who really all bets are off if you have discomfort between the nose and the navel is women women don’t have that classic middle of the chest substernal chest pain women can have shoulder pain they can have upper belly pain they could have any of these kinds of pains and it’d be a heart attack and that doesn’t mean every pain you have is a heart attack it just means that you can have weird pains and I’ll tell you a story from the emergency department I had a sweet little lady come in when late one night she couldn’t sleep because her shoulder was killing her and she was very stoic the lady she was not a complainer at all she’d never been to the emergency department before and so she had no injury to his shoulder but she so convinced me that she had a rotator cuff problem in her shoulder that actually injected her shoulder to try to give her some relief so she could go home and get some sleep and after 20 minutes she said no it’s no better it’s still killing me at that point I as a young ER physician said you know maybe we should get a chest x-ray on this lady she was having a heart attack and so thankfully we didn’t dally too long and we were able to get her to the cardiologist and she’s still with us today but as a young ER doc I missed it she totally convinced me that it was her shoulder and actually when I first suggested it an EKG she said oh I don’t need that it’s not my heart it’s my shoulder but it was her heart and so ladies out there if you have any discomfort between your nose and your navel that bothers you you need an EKG you need to go see your doctor you need to go to the emergency department if it’s severe enough to make sure you don’t have a heart problem so that’s really chest pain in a nutshell I hope this has helped but let me end this video again by saying if your having any discomfort in your chest that’s getting worse that you’re not sure what’s going on you need to go see your doctor okay I personally suffered from from reflux for years and I can tell you personally it is very very severe and remember how I talked about we’re not wired the same in our chest as we are in our fingers it can it can hurt here and it’d be several different things but if it’s hurting here then it’s severe you     I have been having all body muscle twitching for three weeks now it’s mostly in my chest and I feel it almost by my throat . It’s like a jolting feeling and some pressure . It’s so annoying and I worry because it’s close to my heart . But I’ve had ekgs done many actually and it’s normal. Idk what to do I sleep a lot I’m always hydrated and I stay away from coffee . Stress ? Yeah I get stressed but even when I’m relaxed and happy I still get it so .

Don't you think that's the height of being insensitive and silly ? Simply make a short video and only highlight the points first and then keep explaining them each in detail later ?     I've got never ending chest pain and shortness of breath for the last 7 days. It started when I was gardening and breathed in a dust cloud that was coming from dry soil I was raking. The symptoms are similar to the serious symptoms that this video says I should worry about. The pain also gets worse with exercise and radiates to my shoulders and arms.

    I'm very perplexed. I'm in my mid twenties and never got these symptoms before breathing in the dust cloud. What could this be?

    I have no health insurance make $12 an hour i am FUCKED BIG TIME. i have pain in the chest now in the center like when i get up or exiting a car i feel pain then it goes away. Yea i can go to the Emergency room they just bill me $5000 to $14000 for a few hours in the ER blood Ekg Chest xry or CT scan then they will tell me am ok get out when am not i already owe the hospital $14000 from 6 months ago

    I had a heart attack 3 weeks ago, I was tilling my garden. Pain was in both arms thinking muscle soreness. Two days later I went to the ER I was having a heart attack. I had 90% blockage in the widow maker. A stent was put in. I was put on a statin Dr Berry could you do a video on statins ?
    I believe these are dangerous drugs.Risk's to the Liver, Kidney's, lower cholesterol , increase blood sugar or give a non diabetic type 2.

    Im currently symptomatic. I'm experiencing 7/10 chest pressure I upon a medium level of exertion, with dyspnea. I'm having 2-3mm horizontal depressions, and my echo confirms left ventricular wall hypokenesis. It takes about 30 minutes to recover post-excercise and I usually need to give myself a shot or two of NTG spray.

    I had that same feeling of pressure feeling, short of breath I went to the emergency room They did a blood test, ECG, all fine. I was having anxiety prior and I think that was a panic attack. That's what they said. It's really hard to distinguish an anxiety/panic attack from an agnia or heart attack. I felt like the for days dr said all fine and now it's gone

    I’ve been to a cardiologist he did an ultrasound says valves work good and theres nothing to worry about he believes i just have acid reflux, but yet later i still get like a shortness of breath/ lightheadedness when i stand along with chest tightness too.

    With all due respect……at about 5:17 into this video you're talking about the squeezing..bear hug..elephant on the chest feeling…and suggesting to take it seriously and to go see your doctor………..personally i would think that advice could cost a life. I think the right answer is to not waste a single moment and to immediately call ambulance/EMS or whatever emergency service is available in your area.

    Hello I’m 25 yrs old, I am a diabetic type 2 i take insulin and all the medications. I been having chest pain where my left arm gets tingly and numb. I try to ignore it , but lately my heart rate has been 185 bpm and two days ago it was 145. I been super lightheaded ready to pass out. I’m going to the cardiologist tomorrow.

    I'm forty, was just diagnosed with psoriatic arthritis (but must have had it for years, I've lost 3 inches in height 🤷‍♀️, can't wait to see the Mri results) I've learned that the "shoulder pain" threat I've had for years in the middle of the night, is variant angina. This is month 9 of prednisone and I had my first episode in the daytime last week, it radiates from my shoulder through my chest, into my neck, jaw and back of throat? I've never experienced so much pain before. But I'm always in the ER, pneumonia, kidney infections etc and I hate the idea of going. When I move im soaked in minutes and cant catch my breath.

    I recently had blood work, very healthy across the board, been on carnivore for 1 year. Lately I've been burping a lot, so I've been taking ACV for the last week along with re-introducing eggs, and eating quite a bit of extra fat scraps. For the last 3 days I've had minor chest pain. Can't afford to go to the dr. Think to much fat, acv or eggs could cause this? Thanks-

    My dad just suffered a heart attack. All men on my side of the family have heart attacks. I'm thin, so is everyone else. I've eaten terrible my entire life. I'm 32. Ive been getting chest pains the last year or so but more pin pointed pain for a few days. But. There has been pressure the last couple days, mainly due to stress. Feels like an panic attack. Pressure, uneasy, breathing feels hard.

    Thank you so so much doctor. I have the longest time in my life having pains in my upper middle in between my ribcage pain , it feels like a not pressure , if I am stressed or eat heavy meals too fast it get so bad to the point I loose my breath at times. Also my chest start hurting along with my back it almost feels like I am hollow full full of gases even if I just ate . Till my not feeling is gone I get chest pain terrible back pain

    Scary thing I never knew was after lifting weights I got really bad chest pains and I got scared and I went and the drs said it was a inflamed chest muscle and told me to continue to work out to get your body used to the workout

    Okay Dr Berry I’m gonna throw you a curve ball. I’ll try to keep the back story brief. Born in 1969, I’m 50. In 2008 I was diagnosed with Sarcoidosis that was cutaneous. I had a bunch of sores that wouldn’t heal on my scalp… biopsied… Sarcoidosis. CXR fine. Family history of Rheumatoid arthritis, from my mom to my twin sister. CT a few years ago of my heart… my dad passed at 48… I’ve been taking statins since I was about 25-26…. everything is good with my heart, no calcifications. My score is zero, however they have a problem with my lungs. I have a bunch of nodules. My doctors assume it’s sarcoid, no treatment no biopsy. They don’t want to put me on any medication until I literally can’t breathe. I do get short of breath doing activities, carrying laundry up the stairs… I do recover after a few minutes. I also have a 4.3- 4.8 aortic aneurysm per Echo and CT scans. So to get to the pain issue… I’ve had this really intense crushing pain around my diaphragm and in between my boobs. I do have reflux, I’m taking Prilosec once a day, added Zantac 2 times a day. They think it’s reflux, I still have the pain. The cardio thoracic surgeon felt that it’s not a heart problem. The pain when it comes on is so painful, it just gets more and more intense. If I drink ice water, most of the time it goes away. These episodes happen when I’m doing nothing or asleep. Maybe it’s prinzmetal’s angina…. I don’t want to ever bring anything up with my Doctor. I pick and choose if something has been really bothering me and painful. If I mention it, most likely it’s been going on 6-12 months…

    Well… Crap….

    Based on this doc I probably had a heart attack a few weeks ago…. Was walking home from getting some groceries when suddenly I was out of breath and chest pain. Had to stop for a bit before I could continue home.

    Note I'm in a foreign country and don't know what number to call. (Yes I know I should have/should right now look it up)
    I'll be back in the USA next month.

    This is a great video, thank you. My boyfriend was recently diagnosed (about 5-6 months ago at 19) with diabetes and he has been having chest pains, but when he goes to the hospital they rule out anything serious. Because of this, he needs to get it checked by his family doctor or a cardiologist, but he is between doctors as his old doctor was too far away with him needing to get prescriptions all the time for his test strips, insulin etc,. So in the meantime, I decided to do some research and posted on r/diabetes, where someone told me to check this video out. It was really informative and I feel it is super important for me to know this as a woman and girlfriend of a diabetic. I actually may have to go to the doctor too to get my chest checked out as I have discomfort/problems with breathing but it has been part of my life for so long I just calked it up to bad lungs, but this video made me realize that I might want to get it checked out, farther safe the sorry and with women having different symptoms….

    I'm having chest pain right in the middle of the chest for more than 2 months. I went to several doctors to see the problem. Most if them said it is bloating. I thought he would talk about it. I am aware that I have some troubles with bloating and colon, but I do not know how this can be linked to my chest. I went to a cardiologist and did all that stuff and he said no problems found. I'm still having that feeling now. A brief chest pain that can be felt once and disappears. When I move or stand up when I'm laying I have that pain and I can't have it again in the same place. When I touch it's normal, no pain. Nothing. I can deep breath and cough without any feeling. I do not know what to do, still trying to be convinced it's bloating. Anyone like me here ?

    I’m 17, and my parents won’t let me seek help. They think it’s nothing.
    Left side of chest, left shoulder blade, I don’t know what’s happening.
    I haven’t done anything.
    It doesn’t feel better when I exercise. Sternum hurts as well.
    Shooting pain consistently.

    I've had 3 open heart surgeries from newborn till age 4.. also a rare stomach surgery at birth as well. I've had acid reflux my whole life… I get dehydrated fast in summer etc. After I had my son my l4 Nd l5 narrowed into my disc and I have a herniated disc. In the last 6 months I wake up with numb hands but dr told me its probuably a pinched nerve in my neck from my back issues 🙁 … but today I actually had a charley horse kinda pain in my left arm for about 5 mins but i cleaned a ton yesterday… (also pre diabetic & have horrible anxiety) my fear is to have another heart surgery…I've been thru so much I dont remember the surgeries I just remember all the drs I had to see every month growing up. Think this arm pain is serious? I'm on pain killers for my back but i forgot to take it yesterday night and I woke up with back and neck pain (normal) but the arm thing scared me.

    Hi Doc, I feel the need to pop the “sternum” cause of the pressure and now its becoming painful. I feel the pain spreading over the chest. I’ve run a mile or two just to test out the hart at full speed and it doesn’t seem to be it. I’ve suffered from reflux and pop a reflux pill or a bit of baking soda or two tee spoons of apple cider vinegar diluted in grape juice and it calms down: when its reflux. Any tips for the heavy cracking in the middle of the chest?

    I have a cardiologist….howeer a month ago I had chest pain and my heart rate dropped in the 30s and went to er and my cardiac enzymes were very high…I had to be rushed to another hospital. It turned out being microvascular angina caused by my lupus and dermatomyositis…they had to give me atropine twice….had areteriogram…I'm on Ranexa and still having chest pain and and heart rate drops almost every other day…I see my cardiologist wed, Do you know anything about the microvascular heart disease? I also lose my breath….I have to stop 3 to 4 times when bringing one bag of groceries in…

    I've been having chest pain for 5 days now Doctor. Should I worry? I don't have any heart attack symptoms, and it seems to hurt when I twist or make certain movements. Also, rarely I feel pain but very very minor pain, almost nothing.

    Hello! I have a question, I have a 4 year old daughter and yesturday we were coloring on her book when suddenly she start having pain on her chest, she start crying and say Mommy and hurts her lips turn white and she kept complaining about the pain, I put my hand on her chest and she was having really strong palpitations and than she vomit it. scare me at the minute but after she vomited she said she was feeling better. should I worried about that?

    Can anxiety give you the typical heart attack symptoms? I have been to diagnosed with mild depression and anxiety and was given antidepressants, but I still feel like my chest is caving in on the left side as well as shortness of breath and the occasional light headed/dizziness feeling. I have had an ekg and everything clocked out fine, so that is why I’m curious as to whether anxiety can trigger these symptoms

    I had a squeezing pain under my left arm, no big deal. The paramedics said I was not having a heart attack but I went to the hospital anyway. After 2 hours in the er ii was admitted and had another stent put in. I know my body better than anyone. I have had 6 heart attacks in the last 9 years, 3 surgeries and 3 stents. I am 70 and plan to continue on for many years to come. The keto diet has helped me a lot.

    doctor, i have a very pain right on my heart position last night, when i lie down playing with my phone, the pain is so hurt but i still can breat. . i feel like my alteries is blocked, but i not sure, the pain is about 40 seconds and it's goes away, i thought i will die. . . is this a heart attack? because i rarely excercise and always at front desk doing my job 12 hours a day. .. . and now after 16 hours i still can feel a little bit pain on that chest.. FYI this is the first time it happend…

    Hello, I think I went through an heart attack or pre-heart attack 3 weeks ago. I couldn't stand up and was sweating so much. Had also pain on the heart place and the arm on the side. I've had fatigue on both arms and in general and also indigestion since 5 months now. What tests should I ask at the hospital?
